Please choose from the list below to download the registration, ordering and feedback forms for the pre-employment training programme.

If you are a new provider, please complete the trainer registration form for each person who will be delivering the programme.

All providers need to complete the materials order form for each order placed, and the programme registration form with programme delivery dates for quality assurance purposes. All providers will also need to complete the certification request form when candidates have completed the programme.

People 1st will also require copies of the completed feedback sheets for each candidate – if you use your own form, please send copies of these instead. These sheets need to be returned with the certification request form in order for certificates to be sent out.
